Your Role:
As a Care Coordinator at The Good Shepherd, you will play a critical role in supporting our patients and care team. Your responsibilities will include:

  • Patient Communication: Serve as the primary point of contact for patients, providing clear communication and support through in-person, phone, and electronic means.
  • Care Coordination:  Coordinating community resources and referrals, prior-authorizations, and health risk assessments, assisting patients with community resources
  • Documentation: Accurately maintain patient records, track appointments, and ensure confidentiality in all interactions.
  • Team Collaboration: Work closely with the RN Care Team Director, primary care providers, and other healthcare professionals to deliver comprehensive care.
What We’re Looking For:

  • A Registered/Certified Medical Assistant REQUIRED
  • Excellent customer service and communication skills.
  • Ability to work collaboratively in a team-oriented environment.
  • High degree of personal and professional discretion and integrity.

Compensation: $17.00 - $20.00 per hour
